Note that these errata been corrected in the print edition on December 24 2018.
The Gaussian elimination algorithm described in the book has two flaws: (1) it does not order matrix rows with negative coefficients properly and (2) divides by zero when there are coefficients with a zero value in the non-zero half of the triangular matrix.
